CoS annual salary and employer letter for Tier 2 extension

I am applying or Tier 2 extension.
In the Tier 2 main applicant application, the following information is needed

Gross annual salary (not including allowances)

Annual allowances (if any)

My CoS states the gross annual salary (including London allowance) and also states allowances separately.

My contract extension letter states annual salary (not including London allowance) and states London allowance separately.

My questions are:
1) In filling the gross annual salary (not including allowances), which salary should I put in, the one on the extension letter?

2) Is separate letter from employer needed for my salary confirmation and contract dates?

1. the salary without LA at M11. The LA at M12
2. no

read the help text for M11/M12 at the end of the application form

Proof of working

Hi Manci,

I have been working for my company since 2 years. Got the COS and going to make tier 2 application very soon.
Bit confused over providing the salary slips along with application. The supporting documents section in application form says the evidence of proof of working should be provided i.e pay slips for full period.
So should I provide payslips of 2 years ?
