My current visa expires on 31.01.13 and planning to send my application by post on 03.01.13. I was going through the new application form and there is a question (section B, question b12 - b14) asking address details for the last 5 years. But there is space for supplying only one address. In last 5 years, I changed 5 addresses. Do I need to include a separate sheet with all the address details? Do I need to supply any address proof (I don't think I have all these
