Tier-1 Dependent - Different forms (same version 10/2012)

Hi,

I have come across some changes in Tier-1 Dependent application form under same version. Please advise me if anyone has come across this and advise if this would be problem for approval.

When I applied for my dependents in Nov 2012, I downloaded the form from UKBA website which on first page read as below:

[TIER 1, 2 OR 5
PBS
DEPENDANT]

and at the bottom of page -1 it read:
[This form is
specified for
applications
made on or after
1 October 2012]

Now, on UKBA website I find a form where on first page it reads:


[TIER 1, 2 OR 5
PBS
DEPENDANT
version 10/2012

(Difference in title of the form is bold / underlined above)

bottom of page-1 in new form reads same as previous form.

Other difference I find between these two forms is on page 34. This page is blank in previous form. In the new form, page 34 has section Q to list supporting documents included with the application.

I have submitted application for my dependents with the form available at that time i.e. without Section Q. Would this be a problem / reason for rejection.
Current status of application is: I received letters to get their BRP done. BRP has been completed at post office and now waiting for decision.

Please advise.
(I do not know how to attach file or include images in this forum. But if possible to do this, I can attach both forms).

Other related question is on page 34 in Table-A it mentions:

'Evidence of care arrangements if you are under 18 years old'

Please advise what document is required for this? In my children's applications (both are under eighteen) I have attached a consent letter signed by me and my wife to support their application. But not sure which document is expected as evidence of care arrangements for children.

Please advise.

Yes, now this is the form available on UKBA website. But when I downloaded it from UKBA website in Nov 2012, it was different. It seems that they have replaced the previous form on website with this new form without change in version. Hence, now I do not find the previous form on UKBA website and not able to provide the link.
The one I downloaded in Nov-12 and used in my dependent's application also mentions version as 10/2012 on every page. Is it possible to include the attachments to this forum or include images? If yes advise me how I can do this. That will help to show both the forms.
