urgent help required in filling the HSMP application form

While filling the form under point "3. Previous earnings from your work"
"Please ensure that you also provide contact details for each previous employer. This should include the name and address of each employer, a named contact to confirm the information and the telephone number, fax number and e-mail address of the employer."

Does this mean to send the details of the employer under which u were previously employed before your current employer.
I am showing the period of earning for the last 12 months does still i am required to show the details of my previous employer for which i have worked two years before.
please suggest me what to do .

The contact details are to confirm your salary (earnings) details. Therefore, if your earnings period for the last 12 months are derived from more than one company, then will need to provide the contact details for each company for which the earnings were derived.
