EEA4/EE3 Application

Needed some advise and clarification on here guys,i and my EEA Family member already exercised treaty, being living in the UK over 5 years,my questions are.
Is it possible for us to apply together on separate form and sent to same address?The EEA3 or EEA4 address,the reason is that all our documents,other than P60's bears both names.

yes you can.
send both form and documents on eea3 address.
first they will decide eea3 and then pass all the documents for eea4
