Tier-1 General - 2nd Extension

Hello .... seeking advice on this subject.

My (and families) 2nd extension is due this August 2013; queries as below -

1. Is there a minimum period of days beyond which one can't be out of UK - to be proved as part of extension application?

2. under the PBS, do I need to secure min 95points to be eligible?

3. is the application form & fees undergoing change after May, 2013?

4. request if URL for latest application form can be shared please.

1) No.
