Very urgent Tier 1 extension dependant query

Hi Seniors/Members

I am applying for extension tomorrow , I am confused for what care arrangement evidence needs to be provided for my 4 year old child ?

Page no 34 on dependant application form asks for "Evidence of care arrangements if you are under 18 years old".

Secondly I got my bank statement for the rest of the days for as the cycle is between 22nd to 24th every month for maintenance fund etc . They have printed statement on Bank letter head but refused to stamp it. Is that Ok ?

I am also applying for my child next wk. I dont have any idea wht docs I need to attach. As far as I know people are attaching birth certificate and rental agreements and ofcourse maintenance funds.....
