Tier 1 g extension new fees?

Hi need help with a few questions.

1. What are new fees for tier 1 g ext from apr 2013?
2. Pls if you could provide reqd docs link here in forum?
3. Can we provide photocopy salary slips and with original bank statement? Some of my original slips are missing.
