Tier 2 General Extension

Hi Dear,

I switched from PSW to Tier 2 General in July 2010. initially, Tier 2 General visa was issued to me for 3 years. Now in July 2013, I am going to extend my visa for further 2 years. I am working at same job and same employer.

I am requesting you guys, please help me about my extension of Tier 2 General. I am looking for information listed below:

1) Do i need new CoS from my employer?
2) How early we can assign CoS?
3) Which application form is suitable for this category?
4) How UKBA count 5 years for tier 2? e.g. date of issue of CoS or date issues of Visa.

2) How early we can assign CoS? 3 months before apply
3) Which application form is suitable for this category? use the same tier-2 form
4) How UKBA count 5 years for tier 2? e.g. date of issue of CoS or date issues of Visa. Date on your card
