Covering letter for the PBS Dependent Visa

I am applying as a PBS(Tier 1 General) Dependent spouse visa from India.

I am looking for below :

1. Link to online application of PBS(Tier 1 General) Dependent spouse visa in UK BA Site

2. Covering letter sample/template for the main applicant. I do have the Sponsors covering letter template but I need the one that I as a main dependent applicant needs to submit.
