Urgent -help required -Tier-1 dependent VISA from INDIA

I got my tier-1 extension recently.
Now ,we need to apply for the dependent VISA for my wife from India.
Initially ,when I got my Tier-1 G visa on Mar-11 ,that time I didn't apply for my wife's dependend visa .She was in the UK

last year for around 6 months on a VISITOR visa .
Would there be any impact of this ?

Please provide me the list of mandatory documents .

What are the documents I need to send from my side from UK (salary slips ,bank statement s , ? ??)

What is the maintainence fund requirement ?

Neither are required or asked for an adult dependant application of a T1 migrant.
