Urgent Help- Lost indian passport- needs surrender certifica

Hi Can anybody please help me as I have lost mine & spouse indian passport. We were about to apply for surrender certificate this week. I am still not sure looking at the vfs website what documents I need to submit now & do I need to pay any extra fees on top of normal surrender fees also do we need any proof other than police report.

Any suggestions welcome.
