Search for CIC Files reseat! needing help here :/

ok i payed for a search of CIC files Online and printed the reseat... i kept the top part of it for myself but the bottem part of the ticket says to put in the name / address .. do i fill the bottem part 2 of the ticket ? or do they !?
I tryed calling but it wont take me to a real person .. and i hate those answer machean things ... does anyone know anything about this :?:
