Where to get T1 General Extension Visa Form?

Hi, can any one please point me towards Tier 1 General Visa extension form?
all i could find is Tier 1 General Visa form. Help will be appreciated.

So the Tier 1 General and Tier 1 General Extension forms are same?

There is no such form as a 'Tier 1 General Extension' form, it's just the 'Tier 1 General' form (version 04/13) that is valid for applications made on or after 06 Apr 2013.

Bear in mind that the Tier 1 (General) route has been closed (for just over 2 yrs now) so whatever application that is being made now with the form is regarded as an extension as only applicants that have/had a Tier 1 (General) permit are allowed to apply for this permit.
