probably going blind

Hi,

I am probably going blind or what...i am filling up the form MN1 for my child and somehow i can't see any column to mention my child's first name.

1.4=Title
1.5=Surname
1.6=other names (if diff from passport)-there are no other names for him
1.7= nick name (kind of)-there is no nick name

I am not sure where to mention the first name, please help.

Well my child's name has never been changed or different on different certiricates - so i am not sure what to expain on page 13 then?

The key here which Vinny is trying to point out is that IF the name is different to what is on the passport/birth certificate, EXPLAIN on page 13.

If the first name is Samuel and that is what is shown on the passport and birth certificate you write Samuel in 1.6
