Which form you are trying to fill ? You need to fill an application form for passport which can be obtained from post office. You can send your passport with the application form along with birth certificate (I presume child was born in UK and after you got ILR)

If I remember correctly passport application form is same for everyone and it asks both mother and father information on the form.

Thank you for your reply and suggestion

Standard passport application for for first british passport.
Yes my baby was born after i got my ILR.
so i just fillin the details in the form and attach both my and wife's passport along with bio-metric card and that should be ok i guess from your reply!

Thanks

There is a separate page within the passport form guidance which tells you what documents are required. Also, if you go Post Office Check and Send (waste of money IMO), they can tell you what docs are required.
Please do not send me PM if I haven't sent you one yet.
My ILR, MN1 and kids PP stories.

Simple: both your passports, BRPs, full birth certificate, your wedding certificate and official translation if not in english, 2 baby passport photos one of which should be counter-signed as per explained..

Also form filled by yourself as the father (also as the one with ILR), countersignature must be someone knowing you personally for at least 2 years..

Either apply by post or try fast track service.

No hassle, no complications.. done that a month ago myself, we were exactly in the same condition.

Hope it helped.

One parent passport / BRP is enough. Marriage certificate not required if child born after 2006.

In the guidance and regulation documents the requirements are as Jambo has written . However when I was making my application last month, I have presented the birth certificate and my passport & BRP, after they have been reviewed by the line manager (officer took my documents twice to the other room), I have been asked to give my wife's passport, BRP, our marriage certificate. Then it was the second time the passport officer has gone inside, he has come back and said documents are OK only passports and BRPs shall remain for verification thus suggest pay 3 pounds for secure delivery of their return to you.

Hope this clarifies

I just have one more doubt to be cleared with regards to the following :
"countersignature must be someone knowing you personally for at least 2 years"

The above should be a british passport holder or anyone who is staying in Uk for more than two years and also knows me for the mentioned period?

Many Thanks
Azz
