Covering Letter - format if any

Reading the post I see many people using covering letter indicating the documents provided and many more.

I plan to send my application this month end and appreciate if you could suggest the format, the content that goes into it or in short a format if available.

I'm not sure whether this info is available in any of the posts but if it is, appreciate if you could refer me the link.
