Tier 1 genernal dependent - new born child in India

I am about to apply for my daughter's visa from India. Can some one please help which form need to be filled from India (link would be helpful).

I am providing following documents, is any one missing please let me know.

Covering letter
Bank letter confirm balance from India (3 months)
Employment letter (confirm salary and start date)
My passport and wife passport copy
Marriage certificate
Daughter's passport and birth certificate

Who does not need to be tested?

Applicants in the following categories are exempt from the tuberculosis test requirement:

diplomats accredited to the UK;
returning residents (unless they have been absent from the UK for more than 2 years);
Certificate of entitlement holders (right of abode in the UK); and
children under 11 years old (unless deemed necessary by a clinician).

Hmm interesting, UKBA seems to have recently updated this document.

It was the same link I had mentioned in my other post, which seems to have a slightly modified new hyperlink as well. As far as I can remember, this line was not included previously,

* children under 11 years old (unless deemed necessary by a clinician).

But good that this is clearly documented now, it didn't make any sense to have to get this done for infants.
