Indian Banks that give Bank letter as per format

Most of the banks do not give the letter as per UKBA format for third party accounts. ( For eg., in one post, someone mentioned SBI does not give this letter )
Can you please write the experiences with different Indian banks, and which banks give the letter?

The single largest problem causing refusals is because of banks refusing letters for Third party account applications
So it will benefit the rest of us is successful applicants please inform us of banks that DO write letters as per format
