which is exact canadian immigration form?

My wife and I both are Bachelor of Physiotherapists,
we have more than 5 years of experience and i think i can cross 72 point s for Imiigration to Canada.
But i am In confusion which Form i need to fill, because from web site there are so many forms.....Do I need to apply under Skilled immigrants..?
So please kindly help me...
