Anyone who applied under self-employed category for hsmp?

Hello everyone, I am going to apply for HSMP under self employed category , I am having some problems relating to documents that must be send to HO- Anyone here who applied under self employed category and got a HSMP approval ?

Please reply soon, your help will be highly appreciated
