Tier 1 Dependant

Hi
One of my family friend is in UK and his current leave to remain is Tier(General).
His wife came from Pakistan as dependent when he was student.My friend then switch to PSw and then Tier 1.His wife was still as dependent.
In August 2013 he was given further extension as Tier 1 General.
His wife left the UK in February 2013 while she has valid Tier 1 dependent visa(expiry was August 2013).
SHe left UK due to some family reasons.She is still in Pakistan and want to apply for Dependent visa.They also have a boy ages 2 as well .

Question

1- Does husband need to show maintenance £600 for both of them for 90 days??
2- Will she be given Tier 1 dependent visa valid until her husband visa
3- will she be eligible for ILR with husband?
4- Will her application will be considered as extension of dependent visa or ECO will consider her application as fresh application
5- Any specif documents he need to submit?
