General Tier 1 to ILR and Dependant extension Confusion

Hi,

I will be completing my 5 years of stay in UK by 6th March 2014, I am planning to book an appointment soon for ILR,I have done my Life in UK test as well. My wife who has recently joined me as a dependant also need to extend her stay in UK, Her Visa is also expires on 6th March 2014.

I am really confused about which Application Form to use, I came to know that I have to use SET (O) for my ILR, but what would be the form that my dependant has to use ? (she has not completed her two years in UK) how to book the appointment ? Is that two separate appointment or within a single appointment which form that she has to Fill in

As I have only a little bit of Time left, could anyone please help me to sort this.
