Passport Form Section 4 and 5

Hi,

I am filling my passport form after getting Naturalisation certificate, and I got couple of queries. It would be great help if someone can answer:

a. In section 4 (parents' details) they have also asked for the date of birth, place and date of marriage of Grandparents, and have asked us to fill these details in section 8. is this mandetory?

b. In section 5 they have asked for certificate number and place of issue. Is the place of issue is our council office from where we collected the certificate i.e. where we had naturalisation ceremony or HOME OFFICE LONDON?

c. We got our naturalisation certificate in first week of Dec-2013. I hope there was no restriction on getting our passport done within few months of getting our natuaralisation certificate? I know we have delayed it a bit... just a concern.

Q1: Do I need to fill in the details about the grand parents? I don’t know their details

The section about the grandparents in not required if you obtained BC by naturalisation (see page 12 of the guidance notes). It is required if you have obtained it though your parents/grandparents. Some PO C&S might insist you fill in that section. In that case, if you don’t know, write “unknown”. Just remember what you put in as you might be asked at the identity interview about those details.
b.) Home Office London

c.) No issue with the delay. There is no restriction on when you get your passport so long as you are a citizen.

Thanks Just W for the quick reply...

It seems we don't have to enter section 4 at all, but we have done already. So I'll keep it filled, hope officers won't mind.

Got one more query:

My wife has changed her name both First and Last names, and we have got deed poll done thru legaldeedpoll.org.uk guys. So if we send her natuaralisation certificate + old passport with old name and this deed poll with new name will it be enough? Table C on page 15 of guidance notes says we can send:
. enrolled deed poll
. change of name deed signed in both your old and new names. ( It think we fit in here)
