New HSMP form error

While filling up the new HSMP form, I found following error.

Page 1, under heading 'Payment details', line - 11 Payer Name
box nos. 7,8,9 are not accepting any character or looking unformatted.

Anybody confirm me too?

Yes,

This is a problem.

We can write this line completely by pen to avoid this problem.
I am too not sure what to do.

Yes simple, print it and write with Pen....
