How do I assemble my Application - Please help!

How do I assemble my Application - Please help!

Hi everyone,

I'm having trouble deciding where I should document some important information, either within the HSMP Application, or on a separate document, which may be missed by the HSMP Consultant.

Let me explain: The company I work for was purchased by another company last year, so half way through my 12 month earning period my Payslips, bank statements and tax documents change, but they don't change in a logical way.
For the first half of the year, when I worked for company ABC, all my documents show I was paid by a Payroll processing company. When company ABC was purchased by Company XYZ, my Payslips now show I am paid by company ABC, (which is now a subsidiary of Coy XYZ).

To cut a long story short - I have written an explanation of what happened, and I am getting letters from my employer verifying the acquisition, etc; but do I include my explanation within the HSMP application itself, or in a separate document?
It seems the only place to put this sort of information is in the exceptional evidence section. This doesn't seem like an appropriate place for this type of info.

Any advice would be appreciated!

Just Tick the appropriate ones in the application form w.r.t the evidence

It is better and advisable to explain what happened with your Employer n stuff on an additional page.
Anyways you would be writing an covering letter so you can mention these things there on an additional page.no problem with that.
and incase if you think the case worker might not look,then you can Fax this additional page again after you get your reference number.

If im in your place i wouldnt mess the application page writing this stuff.
