deed poll for name change for BC application

Hello, I wish to apply for name change before BC application. I've been told that I should get Deed poll by NCS.

Have anyone for deed poll before? Can we use the one from online or does it need it to done in front of a solicitor or commissioner of oath?

Also, noticed that following point from VFS website for surrender certificate for surrendering Indian passport about name change...

All applicants who have had a name change after acquiring British or any other nationality should fill in the surrender form using the name mentioned on their last/valid Indian Passport. Applicant should also be aware that the surrender certificate will be issued on the name mentioned of their ‘surrendered’ Indian Passport.

In this case, will the OCI card have the new name as per Naturalisation certificate or surrender certificate?
