ILR - Ancestry - Self-Employed - Letter from Accountant

Hi,

I'm applying for ILR via UK Ancestry in April. I'm a company director, so self-employed in the eyes of UKBA. Most of my income is via dividends, with ~10K/year in PAYE wages. I guess that providing personal bank account statements and PAYE payslips mailed out to me is insufficient? Seems like I need a letter from my accountant, question is what exactly does it need to say? I know it should list the PAYE and dividends received for the year, but does anyone have a template or example?

Also, should I bring along dividend vouchers? I don't have any current, would need to write them up and print them off, so seems like a not very trustworthy source. Do I just write up a single dividend voucher covering the year, or each cash transfer into my personal bank account? Technically the bank transfers are directors loans which are then paid off within 9 months via dividends, but anyway... any advice appreciated!
