Question about filling Form VAF4A

Hi,

Im a British national applying for my wife and second child (the other is U.K Citizen) to come into the U.K from Egypt. My child is two years old and i received citizenship after he was born so this doesn't in-tittle him to automatic U.K Citizenship

My questions are,

Which form do I need to fill in to bring him into the U.K? Do i need to fill in Part 5 of Form VAF4A?

My wife entered the U.K. in June 2003, she stayed in the U.K with me until the end of January 2006. We are now applying in June 2014. So in Part 6 of the Form VAF4A, where it asks in Question 6.1 "Have you traveled to the U.K in the last 10 years? Yes or No" Do i answer yes?

While my wife was here in the U.K, she gave birth to our first child in the hospital. In Part 6 of Form VAF4A Question 6.17 where it asks "Have you ever received medical treatment in the U.K? Yes or No" What would be the answer?

While my wife was with me in the U.K we extended her Visa as a post-grad doctor 4 times during the period of June 2003 and January 2006. We only have the visa stickers in her passport which shows the Visa number but not the home office reference number. The problem now is, when we are filling the online application they are asking about the home office reference number. We currently have none of the previous documents regarding the Visa application so my question is.

How do we go about finding this reference number?
