Need Urgent Help for my Child Application Form

Dear All,

My daughter was born in UK, she is two months old now & we are applying for her visa (as my dependant under Tier 1 Entrepreneur category) inside the UK as we are travelling back home for our holidays soon. While looking at the form, it asks for previous immigration history (section K of the dependant form) & the available answers leads you no where like where did you get your last visa, how did you enter UK & what port ? Also at the end of the form it asks for the signatures, so who is liable to sign definately a two months old cant sign. few questions are as under;

K1; What is your current immigration status?
I have leave to enter / remain for a limited period - Go to K4
I am an EU national / family member of an EU national - Go to K4
I am exempt from immigration control - Go to K4
I have leave to enter / remain for a limited period, but leave has now expired - Go to K2
None of the above - go to K4

K5; Did you have a visa or entry clearance when you came to the UK? (ht)
Yes go to K6 No go to K10

K10; How did you enter the UK? (ht)
By air
By sea
By the Channel Tunnel
Travelling overland via the Republic of Ireland
