Problem with EC - Please help

I have applied for HSMP EC a few days ago. Today officer called me and requested to submit additionally a copy of the HSMP application form I sent to WorkPermits (UK). Unfortunately, I do not have this copy (as there is no instruction in HSMP guidance to make a copy before sending). I contacted WorkPermits (UK) and was told they would not normally be able to provide a copy either.

I wonder if anyone was in a similar situation and would appreciate any advice!
