Name mismatch-pantaiema please help

HI Friends,

I have a name mismatch in the documents which iam providing to HSMP like
in some documents my intials are explained and in some not.
example:
my complete name is- Rahulkumar Vadagur Malathesh (Passport).
Degree cerificate -Rahulkumar VM
birth certificate -Rahulkumar VM
English letter-Rahulkumar VM
pay slips -R V Malathesh
P60-Rahulkumar Vadagur Malathesh
Bank statements -R Vadagur Malathesh

is it ok to mention in cover letter ? or anything else needs to be done.

Hi Arunkm_01,
Get a notarised letter stating that x,y,z names refer to the same person are used as X on ... , y on .. and z on ..

In India successfull applicant had done this on 10 Rs stamp paper, notarized by the notorary.

and also mention that in your cover letter.

Currently iam in UK on workpermit.you have any information how and where we need to get this. Is it same as Affdevit?
