Compulsory to provide 2 sets of docs parents visit visa ???

I am planning to call my parents on family visit visa .

Do I have to fill in two separate application forms?
Do I have to provide two separate sponsorship and invitation letters ?
Do I have to provide two sets of bank statements ?

Please suggest
