Important Question about VAF4A-Appendix2

Hi Members,
my wife is going to apply for spouse visa (settlement visa) from Pakistan. She will come with our few months old child. She has to fill up the form VAF4A-Appendix2 form.

There is a question "On what basis are you going to the UK? Put a cross (x) in the relevant box"

My question is this, that does she has to cross (x) two boxes?

1) As the spouse or civil partner of someone settled in the UK.
2)As the child of a parent who is applying for entry clearance
as a partner or the child who is applying to join their parent
who is already in the UK and has been granted limited leave
as partner granted under the Immigration Rules in force on
09 July 2012.

Because she is coming with our child.
OR does she has to fill up this form twice, one for herself and other one for our child?

OR can she cross (x) two boxes on form?

very confusing for me. Could any one of you has gone through this process so far. Please guide me members, if you know about it.

She need to complete 2 separate forms, one for her and one for your child.
