Change of Company while applying HSMP

Hi All

I am planning to apply for HSMP this weekend . But I already resigned from my current company and joining another company within 15 days. Will it be of any problem .????

Does SSLC marks card (where it is clearly mentioned my DOB)suffice for age proof ?

Because my Driving License is handwritten copy instead of laminated Driving license card .Will it be any problem?

I do have problem in my previous earning section. Just like any other company we have Medical , Car , telephone allowance. These allowance can be claimed either once in a month or in end of financial year

I claimed everything in march (in the end of financial year) hence all previous arrears got deposited in march's salary . hence it shows huge amount of salary, but previous months has lesser salary

So when I consider my earning from previous november to this november , march's salaryslip shows deposit of very large amount .

So what should I consider ??? regular March's salary ? or huge gross amount that showed in salary???

I got my passport copy notarized , but in that notary seal doesnt contain his contact number. Will it be any problem?

is it mandatary to give income tax return proofs ?
I do have all 12 months duely attested pay slips , with attested bank statements . So if I dont want to provide tax return , what I have to write as exceptional consideration??
