Director Name (Applicant name) on the Accounts

My extension is due in next week

1- My accountant has filled my latest year accounts with company house which doesn’t have my name in creditor section. But he said " I can give you the word file of your accounts you can just add your name in creditor section. Is it right? Can I just add my name in director loan section without making any changes to actual company house accounts which have already been filed?

2- In the first year my accountant showed my investment of £15000 as Director loan in short-term liability-Creditor: falling due within one year section and in the second year he showed my total investment of Ist year & second year £15000 + £ 18000= £ 33000 as long term liability Creditor: falling due after one year section . Does it sounds right
