Help needed with Naturalisation application

Hi.

I have few urgent questions and I hope someone is can help as I've tried everything possible to find answers.

1. I entered the UK as a minor on my mother's passport which is lost. My first passport was made in 1990. On the application do I enter the date I first got my passport or when I came as a minor. Also, I don't know the exact date as explained the passport is lost and its been 31 years.

2. I had 3 points on my licence for holding a phone while driving over 5 years ago. And I have 2 police caution around 15 years ago. Do I tick I have criminal convictions

3. I have two police caution, last in the year 2000 and I can't remember the exact date. It's not even on my extensive dbs so what do I do about the date.

4. Can a care manager be my referee?
