HSMP VISA FOR DEPENDANT

Hello,

Can anyone kindly let me know the visa form i need the fill in in-order to apply for my 11 month old daughter. She was born here in London, and we are here on HSMP visa- valid till June 2009. We are planning a vacation to India in July, so need to get this sorted ASAP. Please guide me, please help.

She may either apply for FLR(O) before leaving the UK or apply for a dependant visa when in India. See also New born child visa.
