Which form to choose please help

Hi, I got ILR on Feb 2016 and my spouse is in his company visa in UK since 2016 Jan. now we are planning to switch his visa to mine. But confused which form should we take, could you help please.

Why does your husband want to switch to a spouse visa? This will re-set his immigration clock to zero, putting him on a new 5 year (2.5 x 2.50) route to settlement (ILR).

Under which category were you issued ILR :?:
(Casa, not CR001)
