Leave to Remain (Spouse Visa) Application Questions Help Needed

Hi there,

I am Australian with British dual nationality. My husband (Australian) and I currently live in the USA and I am being transferred back to London for work. My husband has had Leave to Remain previously as we lived in the UK for 3 years before, but his LTR expired in November.
We are applying for this again from the US, however we're a little unsure how to answer some of these questions this time around. Any guidance here would be greatly appreciated!

1. Do you have any shared financial responsibilities? (Required)
We have been operating primarily from one bank account in my name but with a shared credit card. As such, bills and rent are in my name (the lease is in both names), however, for proof of shared payments, could we provide copies of his payslips as well as the bank statement showing his cheques being deposited in to the same account these payments were made from?

2. Will you be living anywhere else in the UK?
We have put a temporary residence of my brother's house, but we will be getting our own apartment when we can. Do we use my name here with just "London" as the address TBD?

3. Where do you both intend to live in the UK? (Required)
Similar to the question above, do we put the temporary house or TBD with me?

4. Travel history
As we have lived in the UK and the US, do we have to list every trip in and out of these countries while we were living there? Or can we do one bulk entry for 3 years etc?

5. Accommodation details: Does your wife have any Home Office reference numbers?
What does this one mean?
