Change of Name on BRP for Tier 1 General spousal dependant

Hello everyone,

I need guidance on the exact form to complete in order to change my name on my BRP card to my married name. My passport currently carries my married name but my BRP doesn't.
I completed the change of circumstances form as advised via this link https://www.gov.uk/government/publicati ... s-form-mcc. I completed the form and posted it alongside supporting documents to the UKVI office in Sheffield as advised on the form and Its been 6weeks and I haven't heard any response.
I called the UKVI contact number and the agent advised me to fill the form online that she was not aware of any address for forms to be posted to. She was also unable to assist with the correct form to fill and submit online.

My questions are:
Which form should be completed and how much is it.
Can anyone please send the link to the correct form to be completed as I am confused and seem to be getting contradictory informations.
