Code 004 while reporting (Flr-Hro app)

Hi Guys,
I have applied for FLR-HRO in February 2019. And also reporting every 2 weeks. Last Friday while reporting, one officer said I have to wait because another officer needs to ask me some questions. Afterwards the officer was saying to his collegue - here is a 004 code.

I was asked some basic questions and last question was - do I want to go back home.

But my application is still pending!

Any idea about that guys? Is it the danger sign of that - my application is refused and HO is not letting me know and next repoting time they would hand me a refusal letter and detain me straightway?

Does HO has to definately send a refusal letter of FLR-HRO application? Or they are not bound to?

Please put some light on this matter.
