Extension of long term visitor visa (2 years)

My wife received a 2 years visitor visa without delay.

The question is whether, if need arises, she could extend it. The bits I've picked up suggest that she could apply to extend it for up to 6 months before it expired. So the visa would last 2 1/2 years, rather than 2 years. I've not really looked into the way this is done (eg does she have to pay for a 6 months visitor visa to pick up where the 2 years visa left off?)

But is there a way she can, depending on my circumstances (eg if I am ill in hospital) extend the maximum length of her single stays to over 6 months. Six months is normally the maximum time allowed. So for example, that she could stay for 9 months instead of 6 months if there was a medical problem I have?

Note also that a two year visitor visa doesn't allow her to stay in the UK for the full two years.

Yes, I know the maximum visit stay is 6 months.

As I said, I am aware that at the end of the 2 years visa, it may be possible to extend it for another period.

But my query is about how long she could, depending on the situation, extend her maximum visit period to over 6 months. This extension would be in the middle of the 2 years visa period. Not at the end of it. For example to cover a medical emergency, occurring when she has been here for 5 1/2 months into the 6 months period normally allowed.
