Documents required for spouse re-apply?

Hello,
My wife stay in uk will be close to 2.5 years. Now we need to reapply for another 2.5 years.
What documents are needed this time around. How do i fill the form. If the form is online does my wife need to fill it herself.
How do i provide the required document to HO,?
Do both of us need to be present at same time while submitting the form?
