immigration health surcharge for skilled worker dependent of NHS staff

Hi

I have a job offer from NHS hospital in the UK and have applied as a skilled worker. I was not asked to pay the immigration health surcharge in the application.

I am applying for my wife and children visa as a dependent
My question is

1. which visa application i should use
2. will my dependent and wife's application need to pay an immigration health surcharge.
