Unmarried Partner Form to Sign?

Hello,

Partner looking to join me in the UK, I am British Citizen. They are Spanish. We are looking for the "Unmarried Partner" visa.

They currently hold this certificate from 2016.

Cambridge English Level 1 Certificate in ESOL International (First)
Grade B
First Certificate in English
Council of Europe Level B2

Will this be enough to submit now while we book the appointment or not?

Hello,

Could someone please help me out with this, I've been looking online and people are mentioning a form that the sponsor (British citizen) has to sign that has 3 parts.. apparently. I can only find this form:

but this doesn't have 3 parts, is there another form that I am missing? I cannot find it anywhere on my application, this is for the UNMARRIED PARTNER Visa Application from OUTSIDE the UK.
