Here is Refused letter :

I have refused your application for a visit visa because I am not satisfied that you meet the requirements of paragraph(s) V4.2 of Appendix V because: You have applied to visit the United Kingdom for 06 days to attend Plab2 exam. I acknowledged that you have provided supporting information including test booking details. You state that you are employed earning 121800.00 PKR (£311.84) per month after taxi acknowledged that you have provided supporting information including payslips, employment letter and bank statement. You state that you spend 30000.00 PKR (£76.80) on living expenses per month. This leaves you with a net monthly disposable income of £235. 04.You state that you are planning to spend 2100.00 on your proposed visit to the UK and no one else is contributing towards the costs. I note that your proposed expenditure is more than 08 times of your monthly net income. This degree of expenditure appears disproportionate to your current economic circumstances. I note the statements made in your visa application which indicates that your spouse will remain in your home country. I acknowledge the stated familial ties to your home country however, given my concerns regarding your personal and economic circumstances and intentions for visiting the UK set out above, I am not satisfied that the presence of your stated family in your home country acts to satisfy me that you intend to depart the UK at the end of the period stated by you or that you seek entry for a purpose that is permitted under the visitor rules. The onus is on you, the applicant, to show that you meet the requirements of the immigration rules based on your overall personal circumstances and that you will comply with all the conditions associated with any visa to come to my decision, I have considered, not only those documents stated above, but all the documents you have provided regarding your circumstances and the information provided. I am not satisfied that you meet all requirements of the above paragraphs of the Immigration Rules for visitors. In assessing your entry clearance application, I have considered all of the documents and information provided against the requirements of Appendix V, and have also taken into account the published Visit guidance. The guidance indicates a number of factors which can help assess if an applicant is a genuine visitor, including but not limited to the following: • their financial circumstances as well as their family, social and economic background • their personal and economic ties to their country of residence The concerns noted in the paragraphs above mean that I am not satisfied that you have accurately declared your personal and financial circumstances, which in turn means I am not satisfied on the balance of probabilities that you are a genuine visitor. While I note the information you have provided regarding (your family/assets in your home country/your previous travel history), I am not satisfied that these other factors outweigh the concerns noted above. Therefore, I am not satisfied that you are genuinely seeking entry as a visitor and that you will leave the UK at the end of your visit. Your application is therefore refused under paragraphs V4.2 (a) and (c) of the Immigration Rules.

TBH, her first visa application would have triggered a concern about her real intention.

Most people have their 1st exams in their country. Why did she decide that she wanted to have that in the UK? She's not super rich to afford that.

Then she decides to come have the 2nd exam in the UK (understandable for this). Coming over for 6 days and coming with a 6 months old baby? How does that look? Is she going to take the baby to the exams? If she's truly entering the UK for only 6 days, couldn't she find someone in her home country to look after the child for 6 days? A partner, extended family? You can argue that the baby needs breastfeeding, but that really doesn't hold water. There are formulas out there. It's just 6 days as alluded.

Naturally, once the parent is denied a visa, they also deny the child. Because the child can't enter the UK by themselves, so pretty straightforward.

She plans to spend 8 times her wages (2/3 of her annual income) in 6 days? C'mon, that doesn't look good.

Where has this money boom come from? And she planned same for her first exam, that would have been way cheaper if she'd done it at home. They start to smell a rat. They don't even need to see the rat. Just smelling it would suffice!
No sin in failing, you just have to try and try again!
