Health care visa renewal

As a Healthcare Assistant (HCA) in NHS band 3, earning a salary of £25,000 with SOC code 6131, my visa is set to renew this October. Do I need to submit my Certificate of Sponsorship (COS) and visa application before July 22, or can I extend my application after that date since I am applying with dependents?
