HSMP Extension using TIER 1

Hi

I am trying to apply for HSMP extension under tier1 and got stuck in Section D Page 17 of the application form which asks for Immigration details. I just want to know which immigration status they are asking about ? Question D1 ,as I obtained my HSMP in UK and entry clearance in UK but my initial student visa in India which one I should enter here ?

and for question D5 what should I enter as it is not clear what they are coming to ask there ?

This is about your initial entry clearance in UK. You need to provide the very first entry clearance and the port/airport your landed at.
