Pls help-urgent-self assesment form before april- 06-2010

Dear all,

My tier 1 visa application date is before april 06 2010. I have my biometrics this week.I am not able to find the self assement form as the links are updated and points differ as my application date is befor april-06-2010.Could anyone please send me the pdf for the old version of self assesment form.
